SHORTEN/CANCEL PASS

If you are going to apply for any other passes (MM2H, Student Pass, dependant to LTSVP, etc) or leaving Malaysia permanently, this is where you can find out how to shorten or cancel your pass.

Submit an appointment request and upload documents via MyHelp appointment system and select Location at ESD PUTRAJAYA only. See provided checklist.

Check the status of appointment request in the MyHelp system.

Upon approval of appointment request, please bring documents and original passports (old and new) to ESD PUTRAJAYA.

Collection of passports upon completion of process.

RP-T SHORTEN/CANCEL PASS CHECKLIST

1. RP-T - Main Applicant
Formal cover letter, explaining the circumstances leading to this cancellation request within 30 days
Original passport
Copy of passport (ID page, current valid pass and latest entry stamp)
Completed IMM16 form (signature is compulsory)
Supporting documents (refer to checklist)
2. Dependents (if any) – spouse, children or legally adopted children under 18 years old
Formal cover letter, explaining the circumstances leading to this cancellation request within 30 days
Original passport
Copy of the RP-T main applicant’s passport (only the ID page and current valid pass)
Copy of passport (ID page, current valid pass and latest entry stamp)
Completed IMM16 form (signature is compulsory)
Supporting document (refer to checklist)
3. Children between 18 to 24 years old, parents, parent in-law and common law spouse
Formal cover letter, explaining the circumstances leading to this cancellation request within 30 days
Original passport
Copy of the RP-T main applicant’s passport (only the ID page and current valid pass)
Completed IMM16 form (signature is compulsory)
Completed IMM12 form (signature is compulsory)
Supporting document (refer to checklist)

Shorten/CANCEL pass fee
